I've had several guinea pigs who've had that. It's like a scab. My vet at first recommends Hibitaine but I found that didn't work. The scabs can be also caused by a fungus. I was than given a form of miconzle ointment. (Please check with your vet on this because it may be toxic.)It cured it in no time and it never returned.
