Are you soaking the foot at all?  I don't remember what I used, but the vet
prescribed an antiseptic wash (diluted with warm water).  Even after I
stopped giving an antibiotic (Trimethoprim), I continued the foot soak twice
a day for about ten minutes for several months.  At first Pralines didn't
like it, but eventually she decided it wasn't so bad since it made her feel
better.
