Yes, you absolutely must spay, once there are tumors her life is at stake,
and almost all girlie pigz get them sooner or later.

It is true that scrapings so not always catch mites to such a degree you 
should not even bother with the scraping. Treat all the piggies with 
Ivermectinf 3 times every three wheeks, Frontline and other fleas 
preparations don't get the mites. While mites can cause some thinning of the 
hair the thinning that occurs on the flanks/ back is almost always due to 
the effects of the tumors, If you treat her once for mites and all her hair 
stats filling in immediately, then mites could be the problem, but if the 
thinning persists at all then rush for the spay. My exotic vet said he 
thought spaying was actually easier than neutering as the incision is up 
where it doesn't get dragged in the bedding so much. The experience of the 
vet doing spays is the most important    The most important thing is to keep 
her from chewing her stitches, ask the doctors for Reglan
to keep down the gasses in her tummy, and keep her off veggies until she is 
completely healed as the gas distend the belly and makes the stitches itch, 
some piggies get a lot more gas from veggies than others.  Ask each vet how 
many guinea pig spays he has done and the success rate and choose that way
