Pat, I was so sorry to hear that Audrey has crossed over the bridge. Please don't be too hard on yourself. 
Most likely, it was her time to go and even if you did things differently, the outcome might still have been the same.

Please take comfort in the fact that she passed with Doris watching over her and that now she is at peace.

I wish that there were more that I could say or do to help ease your pain.
