I was just reading thru all the posts and something that is niggling
me.........Duke also did this, but he had a tiny spur on his toofie, one
that escaped view the first time!  Just a thought.
It really does sound like your vet is trying everything.  Pass along the
wonderful advice you've been given, maybe that might trigger something
the vet may think of?? 
Whee all wish dear Chakalaka a speedy recovery and the girls send him
loving brrrrrrrrrrrr's and big kisses to help him through
