Hi!  I feel bad for poor little Sassy and what she is going through.  It sounds as though Sassy needs another vet visit and this time have the vet culture the urine to identify EXACTLY what bacteria is present.  She can also run another test for diabetes at the same time.  What you might try is Bactrim which is actually a little better tolerated by most piggys as well as a pro-biotiYou're right to switch her from alfalfa to timothy pellets and if you even suspect diabetes, no carrots or any kind of fruits as these are all high in sugar.
   
  Best advice I can give is go to www.guinealynx.com and check out the urinary tract infections as well as the section on diabetes.
